How to Access

Carewest offers many programs and services. Here is some information if you would like to know more about how you or a loved one can access Carewest.

Access Transition Services Coordinators act as navigators to the community services and resources by linking callers to appropriate resources.

Anyone may make a referral by calling Transition Services - family, friend, health care professional or a client. Transition Services Coordinators will refer you to Home Care who will assess you for the appropriate community service.

The client must be in agreement with the referral and consent to an in-home assessment.

Adult Day Support

Participants must meet the day support eligibility requirements. Access to Day Support is through Home Care by calling 943-1630.

Comprehensive Community Care (C3)

Participants must live in southwest Calgary within 30 to 45 minutes from Carewest Sarcee. For more information on how to access the C3 program, call the Program Leader at 686-8110.

Supportive Living (SL)

SL offers extra support to residents such as health monitoring and personal care, along with help with meals, bathing and laundry. There is also 24-hour access to an on-call Registered Nurse.

To learn whether or not you qualify for SL, you first have to be assessed by a Home Care Coordinator who will recommend whether or not assisted living will be suitable for you.

If you are a Home Care client, request assessment from your Home Care Coordinator or by calling 943-1600. If you aren't a Home Care client, please phone 943-1920 for an assessment for Home Care. You have to be a Home Care client or an acute care patient to access SL.

Day Hospital

Referrals can be made many ways. Clients self-refer through Seniors Health One-Line Referral by calling (403) 955-1525 or faxing (403) 955-1514, or referrals can be made through family physicians or Home Care. Once the referral has been made, a member of the Day Hospital team will establish a plan for service. Day Hospital staff will advise the family physician of admission and the treatment plan.

Long-term Care and Respite Care

Specialized Assessment and Treatment and Transition Care and Support

Specialized Assessment and Treatment and Transition Care and Support bridge the gap between hospital and home when you are recovering from a serious illness or injury After treatment in hospital, you may be moved to a Carewest care centres where we help you prepare to return home or to move to another supportive living setting.

Each unit has specific criteria. These programs are intended primarily for people already in hospital. Your care team with work with you to determine if this service is the next step for you and when a referral is appropriate.

Veteran Placement at Carewest Colonel Belcher

If you are a veteran, you may be eligible to access Carewest Colonel Belcher and attached Designated Assisted Living suites at the Seniors Residence at the Colonel Belcher.

Veterans of the First World War, Second World War and the Korean War who served overseas or received a disability pension during that time period may be eligible for placement at Carewest Colonel Belcher.

Eligibility is determined by Veterans Affairs Canada through Community Care Access. Assessment for attached Designated Assisted Living rental apartments is also done through Community Care Access.
